To reveal the weak spots easily i recommend attaching an assassins charm to one of your melee weapons and cast them right before approaching a creature. For a clean shot I would recommend any sort of rifle as it is the most accurate, secondary would be a pistol but never the shotty as you will have to get right up it their faces.

As Konquest mentioned, Assassin's charm helps to reveal weak spots if you are within 20 meters. The Assassin's charm reveals weak spots within a 20-meter range. When I ventured into the forest at night to track nocturnal creatures, the charm made it easy to spot them, even when they were camouflaged by the environment. The weak spots appeared bright and highly visible, simplifying the task of aiming.

Beelzeboar - forest pig.

Everhart Deer - interestingly enough, the second weakspot is actually a neck, not a head.

Weald Bear - almost impossible to score a frontal hearthshot due to the large neck.

Catepelose - horned dog-like passive creature, only one weak spot, like many similar small creatures.

For a heart shot, you should aim at the frontal part of the chest, though this spot is often obscured by the head. Alternatively, you can aim from the side of the body, just behind the front leg on either side.
